Since 1995, ACS has supplied medical office products and supplies to hospitals, medical professionals, schools, businesses, and governmental agencies. These Generic Sign-in Sheets securely collect patient information while fulfilling HIPAA regulations. Positions on the sign-in sheet are labeled 1-27 with peel-off adhesive labels. Columns include Position Number, Patient Name, Appointment Time, Arrival Time, Appointment With, New Patient check column, and Any changes to patient information since the last visit. Patient information is transferred via carbon paper to a master log behind the sign-in sheet. Once the slip is removed, that space is blank and the patient information is protected behind the carbon paper, available only for office use.
