This is so helpful for anyone going into clinically and need to get good at taking a patients H&P. I’ve looked at the book so much that I don’t even have to refer to it to know what I need to ask my patients. Amazing for learning and keeping notes to write up your soap notes after!

I used this during my 1st clinical rotation as a Nurse Practitioner student. The template can not only help prompt the Patient interview and history, as well as provide space for accurate documentation, but also has adequate sections noted for the physical assessment and review of systems findings. I have ordered an additional three for adult rotations, one for OB/GYN, and two for pediatrics! It fits perfectly in my lab coat pocket as well.

Was recommended these by a classmate. I am in my DNP for acute care so these don’t work that great for me personally because my clinical currently is a consulting service so my patients change too often. These would be great for someone completing rounds in an ICU or facility where patients don’t change so soon.

In my opinion they are very expensive to be buying constantly for all rotations, so I would recommend buying 1 and then buying black page notebooks once you get a hang of your note taking. Also the pulse section is much larger than BP section. So writing BP super tiny to make it fit while the pulse section has enough room for 3 pulses is not fun. Otherwise it is super useful and a good guide when starting clinical rotations.

I purchased this notebook for my clinical rotations, and it was a lifesaver. It was so nice to have to make sure that I didn't leave any questions out in my history taking and helped me with my presentations.

Haven’t had a chance to use it yet because I haven’t started the job for which I purchased them yet, but they look like they will be helpful. I’m getting pushed to tell what i think of them, so I’m basing off looks and not use. I bought them for a new job, haven’t started it yet
