We used this cover with a deep four inch steel box - re-wiring an occupancy sensor in our rear shop. It fit well and the openings fit the switch and receptacle as expected. A good value. ~ The Duffian

I used this to add a light switch and power outlet to my tool shed. I needed something relatively tough to withstand occasional accidental impacts from moving things around in the shed. I also wanted the switch to be a decora/rocker type instead of the standard/toggle type because the decora switches don't stick out as much and are less likely to get damaged.This Raco 915C duplex cover is attached to a Raco 192 4-inch square box.Note that the outlets/switches attach to the cover and not to the box and you have to attach the outlets/switches to the cover before you install the cover on the box. You have to remove the screws that come with the outlet/switch and insert them through the front of the cover and then through the hole in the outlet/switch. Then install a nut (package of 4 included with the 915C cover) onto the screw to secure the outlet/switch to the cover.You will very likely need to carefully break off the small ears/tabs on the outlets that are meant for keeping the outlets/switches square with the wall. In my case, for the switch, I also had to snip off the small ears with threaded holes for attaching a typical switch plate cover to the switch.
