This is the extension spline you need to connect your kohler handle to the shower body if it set back too far. It does not come with the shower body or handle.

This PART WORKED PERFECTLY!!! I am reposting a review from another customer! It was a brilliant write up of the problem and the solution.-----------------I had a plumber install a a new Kohler shower faucet handle (and new valve) in my bathroom. While I'm guessing that both the valve and shower handle came with a spacer, this brilliant plumber opted not to use this particular piece. because the handle was essentially only held on with a screw, the square piece that fits into the handle would slide around allowing the handle to twist far beyond where it should be.I spoke to the general contractor about this and he said it just happens sometimes with this faucet and he showed me how to fix it. The 7th or 8th time I had to fix this within 2 months, I noticed that there were little gear teeth on the stem of the piece coming from the valve and teeth in the metal square piece between the handle in the valve. The problem was that there looked like there needed to be some type of adapter piece.Seeing this this item had free returns, I opted to try it out. I measured the inside of the metal square piece where the outside of this piece would go and it seemed about the same.I got the piece quickly, it was a perfect fit and finally my children can use the faucet without wrenching it off it's base. If you are trying to figure out why your Kohler shower faucet handle is so crummy, it might be the installer, rather than the faucet itself. Give this a try. It fixed my problem in less than a minute.

Used for Kholer faucet. Better than all the of the replacement plastic splines used, including the original part.

The description said a 20 point to 12 point adapter. Perhaps I'm wrong, but I believed the points were the number of lines on the spline. This has way more than 12 or 20, even evident by the picture. I thought maybe the picture was just wrong, but that's exactly what you get. You can count the number of notches and see the numbers don't match. If you're ordering by part number, and you know this is what you need, then you'll be fine. It seems to have worked for a lot of people. If you're trying to solve an alternate problem, and need an adapter that goes from a 12 spline to a 20 spline, this will not work.
