I’ve been a medical massage therapist in practice for 30 yrs. The important health questions we need to know are not mentioned. Who cares what lotions clients use? No questions about recent surgeries are you being treated for cancer, or any other serious illnesses? A very small line to list meds!It says male massage form…SoAsking if a biological male is pregnant is not a medical question. Asking if they want to referred to as male or female is a different subject all together and should be clarified. Don’t waste your money in these very poorly designed.
