Works great! Hipaa compliant. Has a covered carbon copy under each sign in sheet and is sealed so no one can see it. Very useful!Has places for: name, time, time of appointment, doctor's name and place to indicate if insurance has changed.Adhesive is very sticky, which is good! Paper quality is good and will allow for most inks to be used.Good quality but on the expensive side. Will purchase again.
