I like the book except for the medical expense pages. Nice to log everything in thus far and I used pencil in one spot. Very nice for long term DX and Tx.

I ordered this book to help keep track of my mother-in-laws health. She is 82 and has type 2 diabetes, CHF, and a number of other health issues. My husband and I recently moved in to help her after the death of my father-in-law. She had been ignoring her own health for months so I ordered this book to have a record of all her doctors, medications, symptoms, etc all in one place. I wanted something not too big so I can take it to her appointments so everyone has the same information so she can get the best possible care. This book will be very helpful keeping track of her drs, medications, symptoms, etc. I would recommend this to anyone looking to keep your health info, or a loved ones, in order.

Tabs, vinyl cover, size and spiral are its best features and why I chose this one. However, 75% of the pages are unhelpful for me. Returning.Tab 1: Personal Info- Healthcare providers (1 pg)- Immunization log (6 pgs)- Medication and supplement list (2 pgs)- medical expense log (20 pgs!)- family medical history (2 pages)- blank pages (15 pgs)Recommend: First page should have space for my name and my info like blood type, allergies, DOB. Don’t need med expense log.Tab 2: Vitals and vision- Blood pressure log (15 pgs!)- Blood sugar log (27 pgs!)No vision or dental records pages in my copy! This section is useless to me. May be helpful for someone with a different chronic condition.Tab 3: Ailment tracker- symptom tracker (48 pgs!)This journal is definitely geared for tracking symptoms. Not helpful for me.Tab 4: Dr Office Visits- doctor office visit log (1 page per visit, 48 pages)This is the only section I’d use.
